Important Info to Exchange with Your Nanny/Babysitter

Important information that could make a difference in an emergency.

Information for the Babysitter/Nanny to have at all times:

 

  • Parents' cell phone, work phones, and the number of a back-up person at work who can locate the parents in case of emergency (if there is
  • Home address-- including cross streets and description of house
  • Child's Full Name
  • Date of Birth
  • Copy of Health Insurance Card
  • Doctor's Phone Number and after hours number
  • The name and address of the hospital the child should be taken to in case of emergency
  • Any allergies or medical conditions (including any medication the child is on)
  • Names of back-ups (neighbor, parent of child's friend) to contact in case babysitter has an emergency and needs to leave the kids with someone
  • The National Poison Control # is 1-800-222-1222.
